FOCUS CKD in Edmonton PCN: Facilitating Optimal Care Using Smart technologies in patient at risk of or with Chronic Kidney Disease
The FOCUS CKD project in Edmonton, London is a collaborative initiative between Boehringer Ingelheim Limited (BIL) and Edmonton PCN Ltd. The project aims to enhance the identification, diagnosis, monitoring, and treatment of Chronic Kidney Disease (CKD) in primary care through the integration of the Abtrace digital health platform within the 5 GP practices, thereby improving patient outcomes. Integrating Abtrace with EMIS, email, and SMS will automate test and appointment recalls, enabling timely CKD diagnosis and consistent annual monitoring. This approach will ensure patients see the right healthcare professional (HCP) at the right time, with the aim of improving patient outcomes. It will also reduce wasted appointments by ensuring tests are completed in advance, making consultations more focused and effective.
Key objectives:
- Improve CKD case finding: Utilise the Abtrace digital health platform to proactively identify patients at risk of CKD and ensure accurate diagnostic coding.
- Enhance diagnosis and monitoring: Integrate Abtrace with electronic health record (EHR), email, and SMS to streamline the recall process for tests and appointments, ensuring timely and accurate diagnosis and annual monitoring of CKD patients.
- Improve patient care and experience: by ensuring patients see the right HCP at the right time and, where eligible, are offered and treated to guideline directed therapy.
- Support clinical and administrative capacity: Leverage Abtrace alongside capacity provision to address limitations in clinical and administrative resources, facilitating more efficient CKD management.
This project aims to operationalise in January 2026 and is anticipated to run for 14 months.
Boehringer Ingelheim Ltd is pleased to collaborate with Edmonton PCN Ltd to use digital technologies aiming to improve the detection, diagnosis, management and outcomes for patients at risk of or diagnosed with CKD.
