FOCUS CKD in Brookside Group Practice: Facilitating Optimal Care Using Smart technologies in patients at risk of or with Chronic Kidney Disease

The FOCUS CKD project in Brookside Group Practice (BGP) is a collaborative initiative between Boehringer Ingelheim Limited (BIL) and BGP. The project aims to enhance the identification, diagnosis, monitoring, and treatment of Chronic Kidney Disease (CKD) in primary care through the integration of the Abtrace digital health platform within BGP and associated practices, with the aim of improving patient outcomes. Integrating Abtrace with EMIS, email, and SMS will automate test and appointment recalls, enabling timely CKD diagnosis and consistent annual monitoring. This approach will ensure patients see the right healthcare professional (HCP) at the right time, improving timely and accurate diagnosis and treatment. It will also reduce wasted appointments by ensuring tests are completed in advance, making consultations more focused and effective.

Key objectives:

This project aims to operationalise in December 2025 and is anticipated to run for 14 months.

Boehringer Ingelheim Ltd is pleased to collaborate with BGP to use digital technologies to improve the detection, diagnosis, management and outcomes for patients at risk of or diagnosed with CKD.
